I have read the conditions and regulations of the program (contained herewith and above) and agree to abide by them. I understand that a refundable deposit of $200 is required to enroll in the Advantage QuickStart Day Camp. 10 Day Packs are to be paid in full ($1500). I understand that the balance is due before the start of each season. On any unpaid balance, there is a late charge of 5% per month.
I hereby authorize HCK Recreation dba Advantage Camps to charge my balance (when due) to my credit card. Advantage Camps reserves the right to use videos or photographs of participants for promotional purposes.
We accept checks and offer payment plans upon request.
